HOSPITAL DESCRIPTION

  • The Philippine Heart Center is a 354 bed tertiary care hospital. There are twenty one nursing units, including 53 Intensive Care Unit (ICU) bed, 24 suites, 56 private rooms, a Presidential Suite, 74 Semi-private rooms, 3 Adult Service wards and a Pediatric Service Ward. Its design having 4 petals in each floor represents the 4 chambers of the human heart.
  • The Philippine Heart Center is essentially a hospital for patients suffering from heart ailments. However, it is equipped to render the best possible care for most other illnesses. It ranks as one of the leading medical institutions in Asia for the medical and surgical management of heart diseases. The center admits more than 12, 000 patients yearly, of whom more than 2, 500 undergo heart surgery. Aside from offering medical services, the center is deeply involved in Cardiovascular Research, training and education and public information. It offers training program not only for aspiring heart specialists but for private and government medical practitioners. It engages in various researches on the prevention and control of cardiovascular diseases and undertakes nationwide information programs on the heart and heart care.

DETAILED JOB DESCRIPTION

  • As Operating Room Nurse:
  • Works as a competent member of the surgical team, performing both the scrub and circulating nurse duties and responsibilities.
  • Provides quality patient care. Appraise and help uplift nursing service standard and nursing care practice to meet new surgical concepts and managerial methods. Assumes responsibility for self-development by completing mandatory trainings provided by the Institution.
  • Work as a competent practitioner, contributing to the development of the multi-disciplinary team, ensuring the most effective utilization of resources.
  • Undertake a full range of operating room duties appropriate to the specialty by providing skilled assistance in surgical role.
  • Assess, monitor and evaluate the care needs of the peri-operative patient, gaining the comprehensive understanding of the patient’s condition and reporting effectively to the appropriate person.
  • Maintain and ensure that patient safety, comfort, dignity and confidentiality remain paramount.
  • Display a professional attitude by developing and maintain good working relationships, communication and teamwork with all disciplines, both within the department and with other relevant department in the Hospital.
  • Support and encourage the concept of team working and development of trust, integrity, honesty and sharing within the multi-disciplinary team
  • Specific Duties and Responsibilities:
  • Operating Room
  • SCRUB NURSE
  • The scrub nurse should be sterile conscious. She must have the ability to think, develop and to anticipate the needs of the patient. She must have knowledge of the anatomy, data of the patient and be prepared for any problems that may arise during the operation. She must have thorough knowledge of surgical aseptic techniques, procedures and hospital standards. She must know the proper positioning, preparation and draping of the patient. She must always maintain pride, devotion and motivation in her duties as scrub nurse. She must have a sound judgment.
  • 1. Checks OR schedule for time, type of operation, anesthesiologist and surgeon.
  • 2. Prepares the instruments and special surgical equipment to be used in a particular procedure.
  • 3. Utilizes surgical aseptic technique in opening and handling of supplies and instruments to be used.
  • 4. Scrub hands and arms according to required time and scrub procedure.
  • 5. Must be systematic, accurate and conscientious. Maintains aseptic surgical techniques.
  • 6. Knows all basic and special (cardiovascular/neurological) set-ups.
  • 7. Assists surgical team in proper gowning and gloving process.
  • 8. Assists in aseptic draping of patients for surgery.
  • 9. Assists surgeon in setting up light, cautery cord/s, and suction tubing prior to surgery.
  • 10. Checks sponge count with circulating nurse before surgery and soon as surgeons are ready to close the operative site. Reports immediately to the surgical team any counting discrepancies before closure.
  • 11. Assists the surgeon all throughout the operation. Reviews and practices surgeons’ individual preferences. Making sure that every movement is properly anticipated.
  • 12. Keeps professional communication with the surgical team.
  • 13. Knows the procedural flow of each operation. Handles basic and special instruments safely and correctly. Arranges instruments according to the routine for given procedures.
  • 14. Informs the circulating nurse about the needs of the team.
  • 15. Knows proper indications and usage of suture materials and instruments.
  • 16. Prepares intra-operative medications which may be used in the surgical field.
  • 17. Maintain aseptic technique in the surgical field all the time.
  • 18. Takes care of any specimen that was removed and passing them to the circulating nurse for proper labeling and sending to the Histopathology Department.
  • 19. Discards all sharps and reports any instruments for repair or sharpening.
  • 20. Performs after care of instruments used including special instruments such as the coronary set.
  • 21. Hands over the patient to the Recovery Room.
  • 22. Cleans the OR and keeps things in order ready for use.
  • CIRCULATING NURSE
  • The circulating nurse carries out every need of the patients and use sound judgment. She must have knowledge of all emergency equipments to be proficient during the procedure. She must have knowledge of proper positioning and can assist in providing correct positioners to patient prior to cutting time. She must have the responsibility and authority to maintain efficient functioning in the operating room.
  • 1. Checks OR schedule for the time and type of operation.
  • 2. Cleans room, arranges equipments and secure all necessary supplies.
  • 3. Tests OR lights, suction machines and all apparatus to be used.
  • 4. Performs assessment of patients before surgery.
  • 5. Verifies signed consent.
  • 6. Ensures patient care and safety at all times.
  • 7. Receives perioperative endorsement and ensures all orders, needed data, medications, supplies and materials are complete prior to the surgery.
  • 8. Transfers patient safely from bed to bed or from wheelchair to bed.
  • 9. Attaches patient to cardiac monitor (ECG, pulse oximeter, non-invasive blood pressure[NIBP]).
  • 10. Assists during intravenous, arterial and central venous lines insertion.
  • 11. Prepares antibiotic, does skin test and ensures proper dosage of all medications given to the surgical field.
  • 12. Performs foley catheter insertion and skin preparation.
  • 13. Capable of manipulating/using equipment needed such as the cautery machine, suction machine, back-up light and defibrillator which are used during the patient’s stay in the OR.
  • 14. Opens instruments and supplies to the scrub nurse, maintaining sterile technique.
  • 15. Anticipates the needs of the anesthesia and surgical teams. Keeps professional communication with the anesthesia, surgical and perfusion teams.
  • 16. Is alert and acts accurately especially during emergency situations.
  • 17. Does proper documentation and ensures that all papers are properly filled out by the surgeons and anesthesiologists.
  • 18. Aware of the time flow of each operation, from the patient’s fetching time to the endorsement to the RR or ICUs.
  • 19. Practices proper communication with the other concerned units of the hospital such as the Cardiovascular laboratories, the Pharmacy, Central Supply Unit, Blood Bank, Radiology Department, Cardiac Graphics, Wards, Recovery Room and the ICUs at all times.
  • 20. Monitors the progress of the surgery and maintains the smooth flow of events before, during and after the surgery.
  • 21. Responsible for getting the blood and blood components needed during the surgery.
  • 22. Documents and prepares specimens according to the hospital’s protocol.
  • 23. Assists the scrub nurse, anesthesiologist and surgeon during the entire procedure.
  • 24. Assists in transfer and placement of the patient in the required position with restraint straps in place.
  • 25. Answers needs of the scrub nurse, surgeons and anesthesiologist.
  • 26. Keeps the OR in order. Stays in the operating theater and attends to all the needs of the operating team.
  • 27. Supervises as needed and coordinates with co-workers during patient preparation, transfer and during emergencies.
  • 28. Maintains strict aseptic technique all throughout the procedure.
  • 29. Secures complete count with the scrub nurse before and after the operation.
  • 30. Renders proper endorsement after the surgery.
  • 31. Keeps room in order after the operation ready for use.
  • TRANSPORT NURSE
  • 1. Punctual in fetching patient.
  • 2. Performs accurate assessment of patients and informs charge nurse, anesthesiologists and surgeons for possible delay of operations.
  • 3. Maintains proper communication with the heath care team.
  • 4. Transfers and transports patients with safety precautions.
  • 5. Receives proper endorsement from the ward or ICU nurse.
  • 6. Transfers patient, including all physician’s orders, blood, medications, materials and laboratory results which are needed for the surgery.
  • 7. Makes complete and accurate endorsement to the RR/ICU/ward nurses.
  • CHARGE NURSE
  • 1. Assumes the duties of the Head Nurse in the absence of the latter.
  • 2. Accommodates surgical procedures as long as manpower is available to cater to the patient’s needs.
  • 3. Coordinates with the surgical fellow on duty (SFOD) on the day’s availability of manpower for proper scheduling of cases.
  • 4. Traffics surgical procedures according to the agreed upon schedule with the surgeons/anesthesiologists /perfusionists and other members of the surgical team.
  • 5. Anticipates the needs of the incoming shift for appropriate patient care.
  • 6. Extends help whenever augmentation of staff is needed (i.e., circulating/scrubbing in an emergency case if there are no staff nurses available).
  • 7. Facilitates just division of duties to all nursing staff under his/her stewardship
  • 8. Monitors orderliness and cleanliness of the unit
  • 9. Ensures patient and staff safety at all times
  • 10. Reports sentinel events to concerned individuals if there are any; reports directly to the immediate supervisor of any unusual occurrence concerning the unit
  • 11. Documents activities done and those that are still for accomplishment (i.e., repair of leaking faucets and change of busted lights in the operating theaters by the engineering department)
  • 12. Monitors the implementation of the OR policies and standard operating procedures
  • 13. Night duty charge nurse accomplishes the following tasks:
  • 14. Relieves staff for dinner or until the end of the operation as needed
  • 15. Set-up OR theaters for the next day’s first cases
  • 16. Pulls out instruments for the next day’s ‘to follow’ cases
  • 17. Makes 24-hour unit report
  • 18. Accommodates all emergency cases (acts as circulator while the other staff nurse on duty acts as the scrub nurse); recalls staff nurses for multiple emergency cases scenario
  • 19. Caters to scheduled early cases of the day (cases with cutting time of before 7 o’clock in the morning)
