Ø Assists in the implementation of various health programs of the LGU in accordance with the program goals and objectives of the Department of Health and the Health Sector;
Ø Provides technical assistance in the implementation of the health programs;
Ø Assists in the conduct of disease surveillance;
Ø Conducts health education and training;
Ø Assists in the supervision and monitoring of the Community Health Team (CHT);
Ø Assists in the conduct of regular monitoring and evaluation of the various health programs for the community:
Ø Assists in the preparation and analysis of reports required by the Regional Office/LGU regarding health programs, clinic and community activities;
Ø Performs Clinical Functions such as: Transport of patient and administer injection/immunization
Ø Observes, records and reports patients' symptoms and conditions and progress of treatment and therapy;
Ø Maintains records reflecting patient condition, medication and treatment.
Ø Structurally organize the local dissemination process from BHS to the core hospital level using health resource augmentation in their respective areas.
Ø Disseminate the following information:
Ø For patients/clients going to the BHS requiring protocol for hypertension and DM refer to RHUS.
Ø At RHU level, all patients referred and walk- ins requiring HPN/DM protocols shall undergo the following:
ü Evaluate the health status of the patients (e.g. BP, capillary test, etc.).
ü Educate on Health Lifestyle.
ü Enlist the patient and provide the old Compack medicines if available and or schedule them for a return visit for free medications once drugs are available.
Ø For those patients currently using Non-Communicable Diseases maintenance drugs with doctor's prescriptions that shall be included in the Health Club.
Ø For those requesting to change drug regiments to the available NCD drugs shall secure first a certificate from their Physicians and shall thereafter be a member of the Health Club.
Ø Perform Roles on Service Delivery Network:
Ø Information dissemination regarding client registration with facility.
Ø Provision on health education, counseling, health promotion, and other related health services.
Ø NDPs shall monitor and report the progress of service utilization of the priority population from the demands generated by the Community Health Teams.
Ø Reports will be submitted to the Public Health Associates in the province.
Ø Submits the following to the Provincial DOH Office:
Ø Properly accomplished Daily Time Record (DTR) and accomplishment reports/journal signed by the concerned DMO and MHO/OIC and shall be submitted every 5th day of the succeeding month to the Provincial DOH Office;
Ø Necessary documents and records for purpose of compensation, program monitoring and evaluation as maybe required by the regional/PDOH Office.
Ø Abide the terms and conditions in the signed Code of Discipline;Conducts regular visits to priority households with health problems.
Ø Provides appropriate interventions on identified health concerns of families/households.
Ø Prepares health status reports of families/households visited.
Ø Assists the MHO in the installation of effective monitoring and evaluation systems for UHC programs for health sector performance, monitoring and evaluation;
Ø Assists in the collection, validation, analysis and evaluation of UHC program indicators;
Ø Participates in the analysis of health related data/information;
Ø Assists in the encoding/updating of data/information in the established DOH Information Systems, and submits health reports/data/information to Provincial DOH Offices/Center for Health Development (CHD);
Ø Coordinates with different stakeholders for the submission of national health data reports;
Ø Enhances and sustain logistics management system;
Ø Participates in data gathering, disease surveillance and response during health emergencies and disasters;
Ø Performs other related functions as may be assigned by MHO/DMO.
Ø Establish disease surveillance and response systems in private and public health facilities deemed necessary to protect the health of the population in coordination with DOH/RESU.
Ø Report notifiable diseases (PIDSRs), health events of public health concern (ESRs) and Field Health Information Services (FHSIS) to the next higher-level epidemiology & surveillance unit (ESU).
Ø Provide assistance in the conduct of epidemic/outbreak and/or epidemiologic investigations by:
ü Provide technical assistance in the rapid containment, quarantine and isolation, disease prevention and control measures, and product recall by.
ü Respect and advocate, to the fullest extent possible, data privacy law while maintaining and preserving public health and security.
Ø Performs other functions as may be assigned by the immediate supervisor or head of office.
Ø Assist the MHO in formulating project proposals for priority health programs and project.
Ø Prepare activity reports of the health units.
Ø Together with the DMO, assist the MHO in developing and implementing management systems towards Universal Health Care (UHC);
Ø Assist the MHO and the DMO in formulating and in adopting a Local Investment Plan for Health in the area;
Ø Assist the MHO through provision of administrative assistance for processing of PhilHealth Accreditation of the health facility, to increase coverage of PhilHealth insurance, in encoding PhilHealth Reimbursement claims and recommend to the MHO mechanisms to ensure appropriate allocation and efficient utilization of capitation funds for health;
Ø Assist the MHO in assessing and evaluating the HRH program and leadership/managerial competencies and recommend appropriate actions.
Ø Conduct health data analysis, monitoring and evaluation activities;
Ø Disseminate programmatic and administrative updates to deployed HRH;
Ø Assist the MHO and the DMO in establishing and maintaining the SDN referral system, in tracking referrals in the area of assignment and within the Inter-Local Health Zone (ILHZ);
Ø Participate in data gathering and response during health emergencies and disasters.
Ø Performs other related functions as may be assigned by MHO/DMO.
