·To interview and assess the clients

·Organize, coordinate with the Government and Non-Government Agencies and facilitate meetings and activities for the children and their families

·Undertake customized and Individual Rehabilitation Plan (IRP) for the children and youngsters; Implement, monitor and evaluate the IRP

·Responsible for the writing, collation, and submission of all reports and justification of children to the Director of the Institution and to the functioning Agency

·Assist the Director in the conceptualization, planning and monitoring of the programs and services

·Assist and contribute efforts to the team which is in-charge of fund raising to sustain the program, services and activities of the beneficiaries

·Undertake Social Work task as may be needed to be carried out in-behalf of the other clients of the Institution who are the elderly retirees.

·Interviews, evaluates and classifies patients to determine their eligibility for social services.
·Facilitation/mobilization of internal resources (within the person and family), external resources (hospital an d community) and available material or human resources.
·Advocacy Role- intercedes in behalf of clients and their families.
·Facilitates referrals.
·Conducts follow-up services.
·Conducts Social Work Research through statistical, Narrative and Psych-Social Research.
·Does specific services such as procurement of medicines/ placement of abandoned patients and pauper’s burial.
·Dissemination of information or advice on hospital issues, problems, programs or national social policies related to health as they affect the patient, families and the community.
·Social casework services such as counselling and guidance/information giving

·Supervises Medical Social Work Department staff conducting interviews, and psycho-social interventions with patients and their families.

·Conducts Psycho-social assessment of patients and their families

· Performs social case management to patients referred with social, emotional, and environmental problems, affecting their medical situation.

·Consult, coordinate, and collaborate with other disciplines to provide holistic care to patients, and their families.

·Mobilize resources to meet the medical needs of the patient; Prepares and submit documents and monthly reports.

·Prepare/maintain case records for documentation purposes and recommends problematic cases for team or staff development and conference.

·Provides information/education to patients and their families

·Provide appropriate concrete services

Performs continuous quality improvement; Performs other related functions as may be assigned.
