• Perform Peritoneal Dialysis at patient's home and wound dressing
• Wound dressing of her diabetic foot / gangrenous foot
• Monitor and record the patient’s vital signs and health condition
• Administer prescribed medications and treatments
• Assist the patient with daily activities such as bathing, dressing, feeding, and mobility
• Maintain proper hygiene and cleanliness for the patient
• Provide emotional support and companionship
• Respond quickly to medical emergencies or changes in condition
• Coordinate with doctors, therapists, and family members regarding patient care
• Prepare and maintain medical records and reports
• Ensure a safe and comfortable home environment for the patient

• Provides competent Physical Therapy Services to medically evaluated patients.
• Assesses, plans and implement progress of rehabilitation needs.
• Performs comprehensive evaluation of patients using standard PT procedures on

history taking and physical examination including manual muscle testing,

postural analysis, neuro-evaluation, measurement of joint range of motion etc

• Offers various physical therapy modalities, such as superficial, deep heat and

cold, as well as hydrotherapy techniques, electrical stimulation, traction and

Massage.

• Offers exercise to increase strength, endurance and coordination for specific

muscle groups of the entire body.

• Evaluate muscle length and perform stretching exercise and soft tissue

mobilization to enhance muscle elasticity

• Restore and preserve joints ROM through mobilization techniques and exercise.
• Interprets to the client and his/her family the implication of treatment consisting

with the action and wishes of the referring doctor and physiatrist.

• Translate all exercises into functional activities of daily living
• Coordinates with the medical consultant and other members of healthcare team

regarding patients status and compliance.

• Checks and evaluates all record of treatment response and progress of patients.
• Maintains a safe environment for patients at each session.
