• Leads in the Management of Supplies, Logistics and Cold Chain Management of the Facility.
• Assist in the technical assistance of the municipal health officer in the implementation of guidelines and policies of the DOH and other government offices.

· Collect, receive, store and distribute COVID-19 and routine vaccines based on national standards.

· Acknowledge receipt of COVID-19 vaccines, routine vaccines and other immunization supplies from the Provincial NIP Coordinator and Cold Chain Manager and update the vaccine control card.

· Monitor and record the temperature of the cold chain equipment based on the existing guidelines of the National Task Force against COVID-19 and National Immunization Program, every day, including weekends and holidays.

· Monitor and record status of thermometer, Vaccine Vial Monitor (VVM), expiry date and physical condition of vaccines and diluents prior to distribution and/or administration.

· Prepare and submit monthly reports on vaccine requisition, inventory utilization, and wastage.

· Maintain cold chain equipment in good working condition and submit vaccine or cold chain inventory report as requested.

· Immediately report any breakdown in cold chain equipment to the Municipal Health Officer and NIP coordinator.

· Maintain and implement contingency plans in disasters and mechanical or power supply breakdown,

· Coordinate with the Provincial Health Office Cold Chain Manager and Staff and will represent in meetings and conferences related to vaccines and supply chain management.

· Initiated Active Pregnancy Tracking and Antenal Check-ups to ensure 100% Facility Based Deliveries and attended by Skilled Birth Attendant to the area assigned thereby promoting safe motherhood.

· Initiated Active house to house tracking of missed child and defaulters to ensure full immunization coverage of children 0 - under 2years of age to the area assigned.

· Assisted in the nutritional needs of children 6-11 months and 12-59 months through the Garantisadong Pambata Program through the provision of Vitamin A and Deworming drugs whenever applicable.

· Provides one on one Family Planning counseling by presenting different family planning methods to promote volunteerism and informed choice to women of reproductive age.

· Actively assist in the conduct of implementation, monitoring and evaluation of health programs.

· Assisted in the assessment of Hypertensive and Diabetic clients and referred for treatment whenever applicable.

· Attended during feedback backing to LGU-staff and DOH-staff regarding data finding/issues in health service implementation.

· Conducts health education to patients and Barangay Health workers about various DOH Programs.

· Coordinates with the Public Health Associates to ensure timely data collection and reporting.

· Accountable in the monitoring and tracking various health program data reports. Monitors and tracks needed data for use in various DOH programs. Assisted the LGU staff in the formulation, conduct and implementation of various health programs of the DOH through the provision technical assistance.

· Assisted in the identification of target clients for MNCHN services (Family Planning and Maternal Health)

· Assisted in the updating of TCL on prenatal/postnatal and update the Pregnant Women data on e-Hi5 Registry.

· Assisted in the updating of TCL and update the Child Health Services data on e-Hi5 Registry.

· Augmented Local Government Units (LGU) health service providers in the provision of MNCHN services

· Assisted the RHMs (Rural Health Midwives) in the MONTHLY consolidation, validation and timely submission of barangay/RHU level MNCHN.

· Assisted RHM/Provide Technical Assistance in the conduct of MONTHLY MEETINGS for MNCHN Data Validation, Analysis and Utilization thru the e-Hi5 Registry.

· Collaborated with MHO and DMO regarding MNCHN findings from the field, for appropriate action to address MNCHN issues and concerns identified.

· Assisted RHMs in the creation & functionalization of barangay CHT & Management teams among areas of assignment.

· Provided Assistance to LGU staff by conducting Family Development Sessions to the assigned area.

· Attended and assisted in the implementation of DOH and LGU-health staff in the conduct of health programs/events/activities.

· Attended during feed backing to LGU-staff and DOH-staff regarding data finding/issues in health service implementation.

· Participates in the implementation of various projects/programs/systems and activities related to Maternal, Neonatal and Child Health and Nutrition (MNCHN) and Pantawid Pamilya Pilipino Program; Provides nursing care services related to MNCHN.
